Janice Marie Kozakiewicz

Mrs. Janice M. Kozakiewicz of Bloomfield passed away on October 3, 2011 at the age of 59. Visiting will be at Levandoski Funeral Home 44 Bay Avenue in Bloomfield on Thursday October 6th. The hours are 2-4 & 7-9. The funeral will begin at the funeral home on Friday morning at 9am. The funeral mass will be at Saint Thomas the Apostle Church in Bloomfield at 10am. Interment will follow at Glendale Cemetery, Bloomfield. Janice is survived by her husband Thomas. Her children Matthew and Kelly Auriemma (her husband Joseph). She is also survived by one brother and one sister and several nieces and nephews. In lieu of flowers donations to Saint Jude Childrens Research Hospital Memorial Program 501 Saint Jude Place Memphis TN 38103 in her memory would be preferred.
